Northeast Center for Youth & Families, Inc. (NCYF) is made up of a team of devoted and highly skilled professionals and paraprofessionals who provide quality care to the individuals we serve. Our agency is led by a passionate and involved leadership team based out of our Easthampton main office.
NCYF opened its doors in 1972 and became incorporated in 1976. We’re an experienced agency with multiple locations which provide foster care services, in home therapy, outpatient therapy, treatment residences , and therapeutic day programs. For nearly 50 years, we have been committed to providing innovative and high quality care to our consumers. Clinical Case Manager/Educational Liaison
Northeast Center for Youth and Families, Inc. has an opening for two full-time Clinical Case Manager/Educational Liaisons (CCM) in Residential Services who will focus on clinical, collateral case management, ongoing assessments, and aftercare. The CCM will also act as the Educational Liaison to support the youths' learning and to ensure their IEPs are comprehensive and meeting their needs in the school environment.

This CCM will act as an advocate for the youth in the educational setting. Availability during school hours, attending school meetings, and being available during afterschool hours to establish relationships with students and provide for needs that emerge is required as well as after hours on-call support to the programs
Qualifications:
- Master's Degree
- Successful completion of required trainings once hired, including but not limited to CPI, CPR, First Aid
- Approved CORI & DCF background checks (preformed by the EEC)
- Approved driving background check
- Valid driver's license and reliable transportation required
